Why start with evaluation?

Fertility concerns can involve female factors, male factors, a combination, or no clear explanation after standard investigations. Evaluation aims to understand the clinical situation and guide next steps. It is not a fixed test package, and having tests does not mean that IVF will necessarily be recommended.

When to seek advice

An assessment is generally considered after a year of regular unprotected intercourse without pregnancy when the woman is under 35, or after six months from age 35. Earlier advice may be appropriate with increasing age, irregular cycles, known reproductive conditions or a relevant medical history. Do not delay a discussion if you have a specific concern.

A history that includes both partners

The consultation may cover menstrual patterns, previous pregnancies, operations, infections, medicines and earlier fertility care. Male reproductive and general health history also matters. Tell your doctor about treatments that could affect fertility. Some issues may feel personal; ask how you can raise a concern privately during the visit.

Female fertility assessment

Depending on the history, assessment may include cycle and ovulation review, pelvic ultrasound, selected hormone tests and evaluation of ovarian reserve. A test of whether the fallopian tubes are open may be considered when appropriate. Not everyone needs every investigation. Ask what a test can answer, how it is performed and whether timing within the cycle matters.

Male fertility assessment

Semen analysis is a common part of the initial evaluation. Collection instructions and the laboratory’s requirements matter when interpreting a result. A result outside the reference range does not on its own provide the entire diagnosis. Your doctor may recommend repeat assessment or a relevant specialist opinion depending on the findings.

What ovarian reserve tests can tell you

Ovarian reserve testing can contribute to treatment planning and expected response to stimulation. It is not a simple pass-or-fail measure of fertility and does not guarantee whether you will conceive naturally. Ask how a result relates to your age, ultrasound and history rather than interpreting one number in isolation.

What happens after the tests?

Arrange a review to connect the results with your symptoms and priorities. A useful explanation covers what has been found, what remains uncertain and whether further testing would change the plan. Options can include observation, treatment directed at a specific issue or assisted conception. There is no single sequence suitable for everyone.

Making testing proportionate

Before agreeing to an additional investigation, ask why it is recommended and what decision depends on it. Discuss discomfort, possible risks, preparation, cost and the time to receive results. If you have reports from another clinic, bring them along and ask which need updating. Avoid purchasing a broad online package simply because it contains many tests.

Supporting an informed next step

Keep a copy of your results and write down your questions for the review. Clarify who communicates the findings and how to arrange follow-up. If investigations have not identified a clear cause, ask what that means for planning rather than assuming that the concern is imaginary or that a particular treatment must work. Fertility evaluation should support understanding and shared decisions.

Prepare for a useful consultation

Bring earlier blood tests, ultrasound reports, semen analysis results and records of previous fertility treatment. A list of current medicines, supplements, allergies and relevant operations helps build a complete picture. Ask which previous reports remain useful before repeating investigations.

Discuss work, travel, emotional support and costs. Request an itemised estimate and ask what may change during treatment. Understand the reason for each recommendation, possible alternatives and who to contact between appointments. You can ask for clarification before giving consent.

Frequently asked questions

Do both partners need assessment?

Assessment often considers both partners because either or both can have contributing factors.

Will I need all the tests listed?

No. Tests should be selected according to the history, examination and decisions being considered.

Does a low ovarian reserve result mean pregnancy is impossible?

A reserve result must be interpreted with age and the wider assessment. It cannot by itself answer whether pregnancy is possible.
